Viagra® (Sildenafil Citrate) is the first of three breakthrough oral treatments for male impotence, or erectile dysfunction (ED), currently available with a prescription throughout the UK. Since its release in 1998, the blue diamond-shaped pill manufactured by pharmaceutical giant Pfizer has become the most popular treatment for erectile dysfunction worldwide, with it being successful in around 78% of men at inducing an erection sufficient for penetrative intercourse. Viagra works in response to sexual stimulation and works by relaxing the thin layer or muscle used to regulate blood flow into the penis. Once enough blood has entered the three corpora (tube-shaped structures) within the penis this result is an erection.

How does Viagra work?
Viagra is in a class of medication known as Phosphodiesterase Type 5 Inhibitors, which improve blood flow to the penis, enabling men with erectile dysfunction to respond to sexual stimulation with an erection.
For an erection to occur several chemical messengers are released from the nervous system, one of these such chemicals is Cyclic GMP. Its action is to relax muscle tissue allowing for the widening of blood vessels which flow into the penis. This causes blood to enter and fill the corpora cavernosa and corpora spongiosum, which causes an erection.

After sexual intercourse has occurred and adequate stimulation has been removed the body releases the enzyme phosphodiesterase (PDE5) which breaks down Cyclic GMP, resulting in the erection being lost. Sildenafil, the active ingredient of Viagra, works by inhibiting the effect of this enzyme, allowing the blood vessels to be dilated for longer which improves blood flow to the penis, helping to maintain and prolong an erection.
What is the correct dosage and how long does it last?
Viagra is available in 3 dosages – 25mg, 50mg and 100mg. Usually a patient will be prescribed the 50mg dosage, with the option of this being increased to 100mg if the results experienced are found to be less than hoped for with no adverse effects. Alternatively, this dosage can also be dropped to 25mg if the desired effects were achieved and/or if adverse effects were experienced.
Regardless of dosage, Viagra remains active in the body for up to 4 hours.
When should I take Viagra
Viagra is to be taken up to an hour prior to sexual intercourse, with no more than one dose being taken within a 24 hour period.
If more than once dose has been taken within this period it is necessary to seek medical attention immediately.
Can I take Viagra with alcohol or food?
Yes. Viagra can be taken with both food and Alcohol. However, it is important to drink in moderation when taking this treatment as this can have a lessening effect on Viagra as Alcohol is known to impair sexual performance. In addition to this, when taking Viagra with food it is important to avoid fatty foods as these may lengthen the time that it takes for the medication to be absorbed by your body.
What are the side effects of Viagra?
As with all prescription medication there are side effects attributed to the use of Viagra. These side effects, although less common than not, are more likely to occur when using a higher dose of this treatment but are usually mild and pass in no more than a few hours. The more common of these include headache, facial flushing and upset stomach. It is also possible that you may experience bluish or blurred vision, along with an increased sensitivity to light.
In rare instances men taking PDE5 inhibitors, such as Viagra, have reported a temporary loss, or decrease in vision. If this side effect does occur, or if an erection lasts more than 4 hours, it is necessary for you to contact your healthcare provider immediately.

Is there any reason why I shouldn’t take Viagra?
Viagra is only for male patients with ED; it is not for children or women. Before you start treatment with Viagra, be sure to ask your healthcare provider if your heart is healthy enough, as Viagra can alter blood pressure.
Viagra is never to be used in conjunction with nitrate drugs. The combination of Viagra and nitrates can make your blood pressure suddenly drop to unsafe levels. You could become dizzy, faint, or even suffer a heart attack or stroke. Nitrates are found in many prescription medications that are used to treat angina (chest pain due to heart disease), including:
- Nitroglycerin (sprays, ointments, skin patches or pastes, and tablets that are swallowed or dissolved in the mouth)
- Isosorbide mononitrate and isosorbide dinitrate (tablets that are swallowed, chewed, or dissolved in the mouth)
