The basics

Understanding HRT treatment

We know hormone replacement therapy can feel overwhelming. Let us guide you through the basics so you can make an informed decision.

hormones

The two key hormones

Oestrogen

Relieves most menopause symptoms including hot flashes, night sweats, mood changes, and vaginal dryness.

Progesterone

Protects the lining of your womb if you still have one. Not needed if you've had a hysterectomy.

womb

Do you still have your womb?

With a womb (uterus)

You'll need both oestrogen and progesterone to protect your womb lining.

Without a womb (post-hysterectomy)

You only need oestrogen, which simplifies your treatment options.

Sequential HRT

A sequential regimen provides you with oestrogen every day and adds progesterone for part of the month. This pattern means you have a predictable, monthly withdrawal bleed. It's usually recommended if you're still having periods or are in earlier stages of perimenopause.

Continuous HRT

A continuous regimen provides you with both oestrogen and progesterone every day, which usually stops bleeding altogether. It's typically recommended once you have reached menopause or haven't had periods for at least 12 months.

The different methods

Choose how you take your HRT

There's no one-size fits-all approach. We'll help you choose the treatment option that fits your lifestyle, preferences and medical history.

Don't worry about choosing right now.
In our consultation, we'll clearly show you the best treatment options, along with their pros and cons. You can then choose the type that suits you best.

Frequently asked questions

More info about HRT

What if I'm not sure I have menopause yet?

Our doctors will check whether you may be in perimenopause or menopause during the consultation by asking certain questions. You can start the consultation even if you're unsure - we're here to help you work it out.

Common signs that you may be entering the (pre-)menopausal stage include:

  • hot flushes and night sweats
  • changes in mood or low energy
  • feeling generally unlike yourself
  • difficulty sleeping or brain fog
  • vaginal dryness, reduced sex drive or irregular periods

If you're noticing several of these changes, it could be a sign that your hormones are shifting.

What are the benefits of HRT?

The benefits of HRT usually outweigh the risks.

Relieving menopause symptoms:

HRT can ease common symptoms like hot flushes, night sweats, poor sleep, low mood and vaginal dryness. Many women start to feel better within a few days or weeks.

Preventing osteoporosis:

Falling oestrogen after menopause can weaken bones. HRT helps protect against osteoporosis by restoring oestrogen levels, which is especially important if menopause begins before age 45.

Maintaining muscle strength:

Menopause can reduce muscle strength. HRT helps support muscle health, especially when combined with regular exercise.

What are the risks of HRT?

The overall risks of HRT are very low, and for most women under 60 with menopause symptoms and no major risk factors, the benefits outweigh the risks. Older studies overstated dangers, and current evidence shows HRT is safe for most women when used appropriately.

Breast cancer:

Combined HRT slightly increases the risk, especially with long-term use, but the increase is small (around 5 extra cases per 1,000 women over 5 years). The risk drops again after stopping. Oestrogen-only HRT carries little to no increased risk. However, you should still regularly check your breasts.

Endometrial cancer:

If you have a uterus, you shouldn't take oestrogen on its own because it increases the risk of endometrial cancer. Sequential HRT is safe for up to five years or up to age 54, but after that, you should switch to continuous HRT to keep this risk low.

Ovarian cancer:

Long-term HRT may slightly increase the risk, but this returns to normal a few years after stopping.

Blood clots:

Tablet HRT slightly increases the risk of clots, but the risk is still low (around 5 extra cases per 1,000 women). Patches, gels and sprays, however, do not increase clot risk and are safer options for those at higher risk.

Stroke:

Tablets slightly increase stroke risk due to the elevated risk of blood clots. Patches, gels and sprays, on the other hand, do not. However, the overall risk is still very low, especially under the age of 60.

Other conditions:

HRT has little to no effect on heart disease risk in women under 65. It does not increase the risk of type 2 diabetes and may even slightly reduce it. Evidence about dementia is still unclear.

In which cases can't you prescribe online?

Our online menopause consultation is designed for women aged 45 to 65 who have menopausal symptoms and want to start HRT, or who are already using HRT (but not for more than 10 years).

There are some situations where we're unable to prescribe online because a face-to-face assessment or further tests are needed to ensure your safety. These include:

  • a history of breast cancer, endometrial cancer or other hormone-sensitive cancers
  • certain health conditions, such as cardiovascular disease, gallbladder or liver problems, or blood-clotting disorders
  • strong family risk factors for cancer or cardiovascular disease

You can always complete our free online consultation to see if we can help you safely. If not, we'll guide you towards the right type of care so you know exactly where to get the support you need.

What is the pricing?

You just pay for the medication you choose. Your prescription and free next-day delivery are already included in the price.

Can I switch treatment if I don't feel better?

Yes. If your current treatment isn't working for you, you can request a change through our online consultation. Just let us know that you want to switch, and our doctors will review your symptoms and recommend an alternative.
