What are conjugated oestrogens?

Conjugated equine oestrogens are a type of HRT tablet manufactured by Pfizer, previously known under the brand name Premarin. It is used to treat menopause symptoms such as hot flushes and night sweats. Conjugated oestrogens are a mixture of oestrogen hormones, unlike other oestrogen HRT methods, which contain one form of oestrogen.

How do conjugated oestrogens work?

Conjugated oestrogens are a form of the female hormone oestrogen. During the menopause, the amount of oestrogen in a woman’s body drops. This is what causes symptoms. It therefore replaces the oestrogen your body no longer produces, helping reduce hot flushes, night sweats, and other menopause symptoms.

What are the benefits of conjugated oestrogens?

Conjugated oestrogens reduce the severity of menopause symptoms such as night sweats, hot flushes, sleep problems and brain fog. It is perfect for women who have a broad range of menopause symptoms that require more versatile oestrogen support. It is also available in several doses, meaning you can take the dose that works best for you.

Are conjugated oestrogens the same as Premarin?

Yes, the brand Premarin is no longer supplied in the UK. However, the generic version is simply called conjugated oestrogens, contains the same active ingredients and is available.

Using conjugated oestrogens

What dose do I take?

Conjugated oestrogens are available in 3 doses: 0.3, 0.625 and 1.25 mg.

Most women start on the lowest dose, which is 0.3 mg per day. Your doctor can then increase it if you find the effects are not strong enough. To prevent osteoporosis, the recommended dose is 0.625 - 1.25 mg per day.

If you have a womb, you must also take cyclical progesterone to protect the womb lining for 12-14 days of your cycle. Your doctor will instruct you on how to take progesterone.

How do I take conjugated oestrogens?

Conjugated oestrogens are a form of continuous HRT. You take one tablet each day at roughly the same time, which will help you remember to take it. Do not crush or divide tablets, as this could affect the way the medicine works.

You can start taking it at any time if you are currently not on HRT or you are switching from another continuous HRT (one that does not give you periods).

If you are switching from a sequential HRT (one that does give you periods), start taking conjugated oestrogens after you finish the course of your previous HRT.

What if I miss a dose?

Take it when you remember, unless it's nearly time for the next dose. In this case, skip the missed dose and take the next dose at the regularly scheduled time. Do not take a double dose to make up for it.

Who cannot use conjugated oestrogens?

Conjugated equine oestrogens are safe for most women to take.

Do not take it if you:

  • are allergic to conjugated oestrogens or any of the other ingredients
  • have ever had breast cancer
  • have a cancer which is sensitive to oestrogen, such as endometrial cancer (cancer of the womb)
  • have any unexplained vaginal bleeding
  • have excessive thickening of the womb lining that is untreated
  • have a blood-clotting disorder
  • have had a heart attack, stroke or angina (conditions caused by blood clotting)
  • have a liver disease
  • have a blood condition called porphyria
  • have bleeding on the brain
  • are pregnant or breastfeeding

What are the most common side effects of conjugated oestrogens?

Common side effects of conjugated oestrogens include:

  • changes in breasts
  • hair loss
  • depression
  • leg cramps
  • irregular menstrual cycle
  • vaginal discharge
  • weight gain or loss

How safe are conjugated oestrogens?

Conjugated oestrogens are safe for most women, provided you get a prescription from a licensed healthcare professional. It was approved for use in the UK in early 2025, but the same active ingredient has been available in the form of Premarin since 1994.

However, as with all HRT medicines, it can pose some risk. Research suggests conjugated oestrogens cause a higher risk of blood clots and endometrial cancer than other HRT methods. Therefore, if you have a condition that increases the risk of these complications, you should take special care or consider an alternative like a transdermal patch. Speak with your doctor if you’re unsure about the safety of treatment with conjugated oestrogens.
